Exemestane falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the situation with Aromasin (exemestane). Your liver uses an enzyme called CYP3A4 to help process this medicine. Adrafinil turns into modafinil in your body, and modafinil can speed that enzyme up a little (a process called induction). In theory, that faster clearance could lower your exemestane levels.

Why that matters: exemestane works by lowering your body's estrogen to help control breast cancer. If its levels drop, you'd worry it might not work as well as it should. I want to be honest though, this concern is theoretical, based on how these drugs behave rather than real-world reports, and modafinil is only a weak inducer. Still, because this is cancer treatment, please loop in your oncologist or pharmacist before combining them so nothing gets left to chance.

About Exemestane

What Is Exemestane Used For?

Brand name: Aromasin

Exemestane (brand name Aromasin) is used in postmenopausal women with estrogen-receptor positive early breast cancer who have already taken tamoxifen for two to three years — it is then used to complete a total of five years of hormonal therapy. It is also used to treat advanced breast cancer in postmenopausal women whose cancer continued to grow during or after tamoxifen therapy.

These uses apply to Aromasin and the generic exemestane tablets. This medication is not approved for breast cancer in premenopausal women.
