Exemestane falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for Aromasin (exemestane). Your body relies on an enzyme called CYP3A4 to help process this drug. When Danshen is taken regularly for a week or two, research suggests it can speed that enzyme up, which means your body may clear exemestane faster than expected. In practical terms, that could mean less of the drug sticking around to do its job of lowering estrogen and holding your breast cancer in check.

Because exemestane is a cancer treatment, I'd rather not gamble with anything that might weaken it. The evidence here is early and a bit mixed (a single dose of Danshen actually seemed to do the opposite), so nothing is certain. Please loop in your oncologist or pharmacist before adding Danshen so they can weigh it for you.

What Is Exemestane Used For?

Brand name: Aromasin

Exemestane (brand name Aromasin) is used in postmenopausal women with estrogen-receptor positive early breast cancer who have already taken tamoxifen for two to three years — it is then used to complete a total of five years of hormonal therapy. It is also used to treat advanced breast cancer in postmenopausal women whose cancer continued to grow during or after tamoxifen therapy.

These uses apply to Aromasin and the generic exemestane tablets. This medication is not approved for breast cancer in premenopausal women.
