Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: acetyl-L-carnitine, a compound your body naturally makes from amino acids. It plays a role in how your cells produce energy.
The capsule also holds inactive ingredients — microcrystalline cellulose (a filler), L-leucine, silicon dioxide, and a vegetarian capsule shell.
Does it work?
Moderate evidence
Acetyl-L-carnitine is rated possibly effective for age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic nerve pain (neuropathy), and depression. That means there's some evidence it may help with these conditions, but the evidence isn't yet conclusive.
The strength and quality of the research vary, so talk with your doctor about whether it's right for your situation.
How safe is it?
Well-documented data
Acetyl-L-carnitine is generally well tolerated in the short term, though long-term safety isn't fully established. The most common side effects are agitation, dry mouth, headache, insomnia, and reduced appetite.
Some people report a fishy odor in urine, breath, or sweat. Gastrointestinal upset, nausea, and diarrhea can occur, though they don't happen more often than with placebo.
In people with Alzheimer disease, psychiatric symptoms like depression, confusion, or aggression have been reported, though it's unclear whether the supplement or the disease itself is responsible. The safety data advises against use during pregnancy.
For breastfeeding, there isn't enough reliable information — talk with your doctor or pharmacist for personalized guidance.
Meds to double-check
Moderate interaction found
Before taking this product, double-check with your pharmacist if you're on serotonergic drugs (like antidepressants), blood thinners (warfarin or acenocoumarol), or thyroid hormone replacement. These carry Moderate-severity interaction risks with acetyl-L-carnitine.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
Consider acetyl-L-carnitine if you're exploring support for cognitive decline, Alzheimer disease, nerve pain from diabetes, or depression and your doctor agrees it's appropriate. Be cautious if you take antidepressants, blood thinners, or thyroid medication — check with your pharmacist first.

Acetyl-L Carnitine 500 mg by Klaire Labs Drug Interactions
HelloPharmacist Interaction Report
Acetyl-L-Carnitine 500 mg by Klaire Labs can interact with several medication types.
The most serious concern is serotonergic drugs — medications used for depression, anxiety, and certain other conditions. Animal research shows acetyl-L-carnitine raises serotonin levels in the brain, which theoretically increases the risk of serotonin syndrome (a condition marked by agitation, confusion, rapid heart rate, and high body temperature) and related blood vessel tightening in the brain.
Read the full breakdown — every affected drug type, severity by severity
Two other Moderate-severity interactions exist: acetyl-L-carnitine may strengthen the blood-thinning effects of warfarin (Coumadin) and a similar drug called acenocoumarol (Sintrom), raising your bleeding risk. It may also reduce how well thyroid hormone replacement works by blocking the hormone's entry into cells.
Altogether, these interactions span 203 individual medications. If you take any prescription medication — especially antidepressants, blood thinners, or thyroid drugs — run it through the checker on this page before you start.

For each ingredient in Acetyl-L Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Acetyl-L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
