Acidophilus D.R. has 4 active ingredients: a probiotic blend containing three live bacterial strains — Lactobacillus acidophilus (La-14), Lactobacillus casei (Lc-11), and Lactobacillus rhamnosus (Lr-32) — plus FOS (fructooligosaccharides), which acts as a prebiotic food source for those bacteria. The capsule also contains one inactive ingredient, hydroxypropyl methylcellulose, a binder.
Does it work?
Moderate evidence
L. acidophilus is possibly effective for irritable bowel syndrome (IBS), infection with Helicobacter pylori, antibiotic-associated diarrhea, and bacterial vaginosis. The evidence isn't strong enough yet to rate it for acute respiratory distress syndrome, and it appears ineffective for atopic disease (like eczema or allergies).
We don't have effectiveness data on file for FOS or the other two Lactobacillus strains in this product.
How safe is it?
Well-documented data
L. acidophilus is generally well tolerated in healthy adults and children. In rare cases, taking it with other probiotics can cause mild stomach discomfort, gas, bloating, or diarrhea, though these side effects are uncommon.
There's a theoretical concern that live L. acidophilus could cause serious infection in people with compromised immune systems, though this is very rare. Pregnancy safety is rated possibly safe; for breastfeeding, we don't have data on file.

L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
