Short-term use of agmatine sulfate appears generally well tolerated, but long-term safety in humans hasn't been well studied. The most common side effects in trial use were diarrhea, indigestion (dyspepsia), and nausea — mild-to-moderate diarrhea and nausea appeared in a few trial participants within 2–3 days and resolved when they stopped taking it.
