Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: acetyl L-carnitine HCl at 600 mg per capsule. Acetyl-L-carnitine is a modified form of the amino acid carnitine, naturally produced in your body and found in some foods.
The capsule itself is made from bovine gelatin, an inactive ingredient.
Does it work?
Moderate evidence
Acetyl-L-carnitine is rated possibly effective for several conditions based on available evidence: age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy, and depression. This means there is some supporting research, but the evidence isn't conclusive enough to say it definitely works for these uses.
If you're considering it for one of these purposes, talk with your doctor about whether the current evidence makes sense for your situation.
How safe is it?
Well-documented data
Acetyl-L-carnitine is generally well tolerated short-term, but long-term safety hasn't been fully established—so extended use should be discussed with your doctor. Common side effects include agitation, dry mouth, headache, insomnia, and reduced appetite.
Some people also report a fishy odor in urine, breath, or sweat, which comes from a breakdown product of the supplement. Gastrointestinal upset (nausea, vomiting, diarrhea, constipation) can occur, though it's not more frequent than in people taking placebo.
In people with Alzheimer disease, psychiatric symptoms like depression, confusion, and aggression have been reported, though it's unclear whether they're caused by the supplement or the disease itself. There isn't enough reliable safety data on pregnancy, so it's best avoided unless your doctor advises otherwise.
Safety while breastfeeding is unknown, so avoiding it is the safer choice.
Meds to double-check
Moderate interaction found
Before taking this product, double-check with your doctor or pharmacist if you use serotonergic drugs—including most antidepressants, some pain relievers, and other medications that affect serotonin. Also flag it if you take blood thinners (anticoagulants) like warfarin or acenocoumarol, or if you're on thyroid hormone replacement.
No interactions are documented for the ingredients we could not check, but that's not a guarantee none exist.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
Acetyl-L-carnitine may be worth considering if you're interested in cognitive support or have one of the conditions it's possibly effective for, and your doctor thinks it's appropriate. However, if you take blood thinners, antidepressants, or thyroid hormones, you'll need to clear it with your doctor or pharmacist first.
Talk with your healthcare provider before starting, especially if you're pregnant, breastfeeding, or taking any regular medications.

ALCAR (Acetyl L-Carnitine) 600 mg by BulkSupplements.com Drug Interactions
HelloPharmacist Interaction Report
Acetyl L-Carnitine 600 mg by BulkSupplements.com has documented interactions with several medication types.
The most serious concern is with serotonergic drugs—antidepressants, certain pain relievers, and other medications that raise serotonin. Animal studies show acetyl-L-carnitine can increase brain serotonin levels, and theoretically this raises the risk of serotonin syndrome (a potentially serious condition with agitation, confusion, rapid heartbeat) and related vascular issues.
Read the full breakdown — every affected drug type, severity by severity
Blood thinners (anticoagulants) are another moderate-severity concern. Acetyl-L-carnitine may enhance the effects of warfarin and acenocoumarol, which could increase bleeding risk.
Thyroid hormone replacement therapy is also affected—acetyl-L-carnitine may theoretically decrease how well your thyroid medication works by blocking its entry into cells.
Altogether, these interactions span 203 individual medications. Before you start this supplement, run your exact medications through the checker on this page, and talk with your doctor or pharmacist if you take any blood thinners, antidepressants, or thyroid hormones.

For each ingredient in ALCAR (Acetyl L-Carnitine) 600 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Acetyl L-Carnitine HCl
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
