Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
This supplement contains 11 active ingredients — six probiotic bacterial and yeast strains, plus herbal extracts. The live organisms are Lactobacillus plantarum, Lactobacillus rhamnosus, Lactobacillus acidophilus, Bacillus coagulans, Saccharomyces boulardii, Bifidobacterium animalis lactis, and Lactobacillus gasseri.
They're complemented by Holy Basil Leaf Extract, Triphala fruit extract, and Fenugreek seed extract, all intended to support digestive and women's health. The product also contains inactive ingredients — Hypromellose (a capsule material), Rice Fiber, and Rice extract Blend — which are fillers and binders.
Does it work?
Moderate evidence
The evidence for what this product can do varies by ingredient and use. Lactobacillus acidophilus is possibly effective for irritable bowel syndrome, Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bacillus coagulans is possibly effective for constipation and irritable bowel syndrome. Saccharomyces boulardii (the yeast) shows possibly effective evidence for rotaviral diarrhea, travelers' diarrhea, antibiotic-associated diarrhea, necrotizing enterocolitis, Clostridioides difficile infection, and Helicobacter pylori infection.
Fenugreek seed is possibly effective for sexual dysfunction, sexual arousal, dysmenorrhea, and diabetes. For Holy Basil Leaf Extract and the other probiotics in this formula, the evidence we hold isn't established — the data rates them as having insufficient reliable evidence for the conditions they're marketed to address.
How safe is it?
Well-documented data
Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Bacillus coagulans is generally well tolerated in healthy adults, though evidence and product quality vary.
Saccharomyces boulardii is generally safe for healthy adults but should be avoided by people who are critically ill or immunocompromised. Rarely, any live probiotic organism can cause infection (bacteremia or sepsis) in very ill patients with central catheters, but this is uncommon in relatively healthy people with intact immune systems.
Gastrointestinal side effects like bloating, flatulence, abdominal pain, and nausea can occur with probiotics but are uncommon. Holy Basil Leaf Extract is generally well tolerated short-term; the most common side effects are loose stools and nausea.
Safety data are limited for long-term use, and the safety data advises against it during pregnancy. There is not enough reliable safety information to use it while breastfeeding.
Fenugreek seed is generally well tolerated in food amounts, but supplement doses may cause abdominal pain, bloating, diarrhea, flatulence, nausea, or rare severe allergic reactions. The safety data advises against medicinal use during pregnancy.
Meds to double-check
Moderate interaction found
Before you take this product, double-check your medications against anticoagulant and antiplatelet drugs (Moderate risk from fenugreek), antidiabetes drugs (Moderate risk from both holy basil and fenugreek), antibiotics (Moderate risk from three probiotic strains), and if you take warfarin, clopidogrel, phenytoin, metoprolol, theophylline, sildenafil, or pentobarbital, those warrant a careful look too — all Moderate interactions with fenugreek or holy basil. Antifungals may reduce the effectiveness of the yeast in this formula (Minor).
Use the medication checker below to be sure.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.

Ancient Probiotics Women's Once Daily by Ancient Nutrition Drug Interactions
HelloPharmacist Interaction Report
Ancient Nutrition's Ancient Probiotics Women's Once Daily contains several live probiotic organisms and herbal extracts.
The most serious interaction we've documented is with anticoagulant and antiplatelet drugs — blood thinners like warfarin and aspirin — due to fenugreek seed extract, which may increase bleeding risk. This is rated Moderate severity.
Read the full breakdown — every affected drug type, severity by severity
Additionally, several ingredients interact with antibiotics (Lactobacillus acidophilus, Bacillus coagulans, and Lactobacillus gasseri). Since these probiotics are live organisms, antibiotics can kill them and reduce their effectiveness — you'll want to space out the timing of these two.
Holy basil leaf extract also raises two Moderate concerns: a theoretical increase in low blood sugar (hypoglycemia) risk with diabetes medications, and a possible boost to sedative effects if you take pentobarbital. Fenugreek seed extract carries additional Moderate interactions with diabetes drugs, the blood thinner warfarin, the antiplatelet drug clopidogrel, the beta-blocker metoprolol, the seizure drug phenytoin, and the erectile dysfunction drug sildenafil — each affecting how those medications work.
Saccharomyces boulardii (a live yeast) may lose effectiveness if you're also taking antifungals, rated Minor.
We could not check three probiotic species — Lactobacillus plantarum, Lactobacillus rhamnosus, and Bifidobacterium animalis lactis — and we also hold no data on Lactobacillus paracasei, Triphala fruit extract, or what's inside the Women's Herbal Blend itself. Altogether, these interactions span 611 individual medications.

Fenugreek seed extract
Anticoagulant/Antiplatelet Drugs
Theoretically, fenugreek might have additive effects when used with anticoagulant or antiplatelet drugs.
Some of the constituents in fenugreek have antiplatelet effects in animal and in vitro research. However, common fenugreek products might not contain sufficient concentrations of these constituents for clinical effects. A clinical study in patients with coronary artery disease or diabetes shows that taking fenugreek seed powder 2.5 grams twice daily for 3 months does not affect platelet aggregation, fibrinolytic activity, or fibrinogen levels .
Antidiabetes Drugs
Theoretically, fenugreek seed might have additive hypoglycemic effects when used with antidiabetes drugs.
Clinical research shows that fenugreek seed can reduce fasting blood glucose and 2-hour postprandial glucose levels in adults with type 2 diabetes.
Clopidogrel (Plavix)
Theoretically, fenugreek seed might alter the clinical effects of clopidogrel by inhibiting its conversion to the active form.
Animal research shows that fenugreek seed 200 mg/kg daily for 14 days increases the maximum serum concentration of clopidogrel by 21%. It is unclear how this affects the pharmacokinetics of the active metabolite of clopidogrel; however, this study found that concomitant use of fenugreek seed and clopidogrel prolonged bleeding time by an additional 11%.
Metoprolol (Toprol)
Theoretically, fenugreek seed might have additive hypotensive effects when used with metoprolol.
Animal research shows that fenugreek seed 300 mg/kg daily for 2 weeks decreases systolic and diastolic blood pressure by 9% and 11%, respectively, when administered alone, and by 15% and 22%, respectively, when given with metoprolol 10 mg/kg.
Phenytoin (Dilantin)
Theoretically, fenugreek might decrease plasma levels of phenytoin.
Animal research shows that taking fenugreek seeds for 1 week decreases maximum concentrations and the area under the curve of a single dose of phenytoin by 44% and 72%, respectively. This seems to be related to increased clearance. So far, this interaction has not been reported in humans.
Sildenafil (Viagra)
Theoretically, concurrent use of sildenafil and fenugreek might reduce levels and therapeutic effects of sildenafil.
Animal research shows that taking fenugreek seeds for 1 week reduces maximum concentrations and the area under the curve of a single dose of sildenafil by 27% and 48%, respectively. So far, this interaction has not been reported in humans.
Theophylline
Theoretically, fenugreek may reduce the levels and clinical effects of theophylline.
Animal research shows that fenugreek 50 grams daily for 7 days reduces the maximum serum concentration (Cmax) of theophylline by 28% and the area under the plasma drug concentration-time curve (AUC) by 22%.
Warfarin (Coumadin)
Theoretically, fenugreek might have additive effects with warfarin and increase the international normalized ratio (INR).
Some fenugreek constituents have antiplatelet effects, although these might not be present in concentrations that are clinically significant. In one case report, a patient taking warfarin experienced an increased INR when starting to take fenugreek in combination with boldo.
Antihypertensive Drugs
Fenugreek may also have an additive effect on blood pressure-lowering medications. Studies on animals have shown that fenugreek seed can decrease both systolic and diastolic blood pressure by up to 22% when combined with metoprolol. Therefore, it is essential to monitor your blood pressure regularly if you are taking fenugreek and metoprolol together or any other antihypertensive drugs.
Holy Basil Leaf Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, holy basil seed oil might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Animal research shows that holy basil seed oil can prolong bleeding time, possibly due to inhibition of platelet aggregation. However, it is not known if this occurs in humans.
Antidiabetes Drugs
Theoretically, holy basil might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Small clinical studies show that taking holy basil can decrease fasting blood glucose and other measures of glycemic control in patients with type 2 diabetes.
Pentobarbital (Nembutal)
Theoretically, holy basil seed oil might increase the sedative effects of pentobarbital.
Animal research shows that holy basil seed oil increases pentobarbitone-induced sleeping time. However, it is not known if this occurs in humans or if this applies to other barbiturates or sedatives.
Lactobacillus acidophilus
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bacillus coagulans
Antibiotic Drugs
Theoretically, taking antibiotics with Bacillus coagulans might decrease the effectiveness of B. coagulans.
B. coagulans preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. coagulans preparations by at least two hours.
Lactobacillus gasseri
Antibiotic Drugs
Theoretically, taking Lactobacillus gasseri with antibiotic drugs might decrease the effectiveness of L. gasseri.
Lactobacillus gasseri preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. gasseri preparations by at least two hours.
Saccharomyces boulardii
Antifungals
Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
