Short-term use of andrographis is generally well tolerated, but long-term safety and product quality aren't well established. The most common side effects at typical doses are mild: abdominal discomfort, altered taste, diarrhea, dizziness, fatigue, headache, nausea, and rash.

These are more likely at higher doses (5–10 mg/kg of the andrographolide content) or beyond 14 days of use. Serious allergic reactions, including anaphylaxis, are rare.

Skin reactions—including rash, itching, hives, angioedema, and rarely Stevens-Johnson syndrome—have been reported. Avoid andrographis during pregnancy because animal studies and theory suggest it may affect reproduction.

There isn't enough safety data to use it while breastfeeding, so it's best to skip it if you're nursing.

Etoricoxib (Arcoxia)

Theoretically, andrographis might decrease the absorption of etoricoxib, although the clinical significance is unclear.
Animal research shows that andrographis extract, or the constituent andrographolide, taken orally with etoricoxib decreases the bioavailability of etoricoxib. However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.

However, the opposite effect is seen with the constituent, andrographolide, in which the maximum concentration and area under the curve are decreased when taken with glipizide.
