Andrographis is generally well tolerated when taken short-term, though the quality of available products and long-term safety haven't been fully established. Doses around 5–10 mg/kg of andrographolide (the active compound) or higher, especially beyond 14 days, raise the risk of side effects.

Common side effects include stomach discomfort, altered taste, diarrhea, dizziness, fatigue, headache, nausea, vomiting, itching, and rash. Serious allergic reactions including anaphylaxis are rare.

Andrographis has also been linked to various skin reactions including Stevens-Johnson syndrome and angioedema, though these are uncommon. Avoid this product during pregnancy because of theoretical concerns and animal-study data suggesting reproductive risks.

Theoretically, andrographis might decrease the absorption of etoricoxib, although the clinical significance is unclear.
Animal research shows that andrographis extract, or the constituent andrographolide, taken orally with etoricoxib decreases the bioavailability of etoricoxib. However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.

However, the opposite effect is seen with the constituent, andrographolide, in which the maximum concentration and area under the curve are decreased when taken with glipizide.
