Black cohosh is generally well tolerated short-term but carries rare liver safety concerns, so medical guidance before use is wise. Common side effects of black cohosh include breast tenderness, dizziness, headache, gastrointestinal upset, and tiredness.
California poppy has limited human safety data; when combined with other ingredients in studies, it's been reported to cause nausea, muscular stiffness, and morning fatigue, though it's unclear whether those effects came from California poppy alone. Glycerol in large doses can cause stomach upset, bloating, diarrhea, nausea, vomiting, and headache.
Black cohosh is rated possibly unsafe in pregnancy and lactation. California poppy, agrimony, mullein, and yucca all lack sufficient safety data and should be avoided during pregnancy and breastfeeding.

Black cohosh may inhibit CYP2D6, an enzyme that breaks down many psychiatric and cardiac drugs, potentially raising their levels in your bloodstream. It also interacts theoretically with blood thinners (anticoagulants) and antiplatelet drugs, estrogen therapy, cancer drugs like cisplatin, and serotonergic drugs (antidepressants and anxiety medications) — all rated Moderate severity.
There's an additional Minor-severity interaction with a class of transporter proteins (OATP) that move certain drugs into your body.
