Moderate evidence
Beetroot has been studied for several uses with mixed results. It's rated Possibly Effective for athletic performance and for reducing exercise-induced muscle soreness.
For chronic obstructive pulmonary disease (COPD), the evidence leans toward it being Possibly Ineffective. For dyslipidemia (abnormal cholesterol and triglycerides) and cognitive function, there isn't enough reliable evidence to rate either way — we simply don't know yet whether it helps.
How safe is it?
Well-documented data
Beet as a food is widely eaten and generally well tolerated by most adults. Concentrated supplements, though, don't have as much safety data behind them, so use some caution if you go that route.
Short-term use for medicinal purposes appears to be well tolerated overall. The most common side effect is harmless red discoloration of your stools and urine — don't be alarmed if that happens.
Rarely, beet supplements have been reported to cause black stools. In very large amounts, beet could theoretically cause low calcium (hypocalcemia) or kidney damage, but this is rare at typical supplement doses.
One case reported a patient on blood thinners (apixaban and clopidogrel) who developed black stools and nausea after taking beet pills; his doctors found the risk of serious bleeding was low, but the link was noted.

In vitro research suggests that betanin, the main pigment in beet, may inhibit this enzyme similarly to a strong inhibitor, potentially raising the levels of these drugs in your bloodstream and increasing their effects.

Beetroot also interacts with CYP1A2 substrates (another liver enzyme category), though this effect appears to work in the opposite direction — beet may decrease their levels and clinical effects. That's a Minor interaction.
There's also a Minor interaction with blood pressure medications (antihypertensives). Beet contains nitrates, which relax blood vessels and can lower blood pressure; taking it alongside blood-pressure-lowering drugs could theoretically amplify that effect.
That said, one clinical study using concentrated beetroot juice found no significant change in blood pressure or heart rate, so the real-world risk may be smaller than the theory suggests.

Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Antihypertensive Drugs
Beet and beetroot contain nitrates, which can cause vasodilation, potentially leading to lower blood pressure. However, a study published in the European Journal of Clinical Nutrition using concentrated beetroot juice found no significant impact on blood pressure or heart rate in different age groups. Other small clinical studies suggest that while beet consumption might transiently lower blood pressure due to vessel dilation, there's no consistent evidence of a lasting effect. Overall, the theoretical risk of reduced blood pressure due to beet's nitrate content exists, but studies generally indicate a low and temporary impact rather than a sustained decrease.
Cytochrome P450 1A2 (Cyp1A2) Substrates
