Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Herbadiet Bilberry Extract is a single-ingredient supplement: bilberry fruit extract in capsule form. Bilberry is a small dark purple fruit related to blueberries, and the extract concentrates its compounds—mainly anthocyanidins, the pigments responsible for its color and potential health effects.
Does it work?
Leans against
The evidence for bilberry extract is mixed and often limited. For night vision, current data rates it possibly ineffective—claims that it sharpens vision don't hold up well in rigorous studies.
For age-related cognitive decline, age-related macular degeneration, atherosclerosis, high cholesterol, and chronic venous insufficiency, the evidence we hold is insufficient to rate whether it works. If you're considering bilberry for one of these uses, talk with your doctor or pharmacist about what the science actually supports.
How safe is it?
Well-documented data
Bilberry fruit itself—the whole food—is generally safe when eaten. However, the safety data for concentrated supplements like this one is limited and incomplete.
The most common side effects are dark-colored or discolored stools, flatulence, and stomach upset; in studies, these appeared in a small number of participants. In one trial, about a third of patients reported mild-to-moderate gas, though that group had ulcerative colitis.
Your tongue, teeth, and stool may turn dark blue or black—this is harmless but can be startling. Because long-term safety information is sparse, manufacturers advise caution with supplement doses.
Meds to double-check
Moderate interaction found
Before taking Herbadiet Bilberry Extract, double-check it with your doctor or pharmacist if you take blood thinners or antiplatelet drugs (Moderate risk of bleeding), diabetes medications (Moderate risk of low blood sugar), drugs broken down by CYP2E1, or erlotinib (Moderate risk of reduced effectiveness).
The bottom line
Scorecard at a glanceFully disclosed formula with graded evidence leaning against its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Bilberry extract is generally well tolerated if you tolerate it individually, but it has documented Moderate interactions with blood thinners, diabetes drugs, and certain cancer medications. If you take any prescription drugs—especially anticoagulants, antiplatelets, or diabetes medicines—check with your doctor or pharmacist before starting.
The evidence for its effectiveness is weak or missing for most uses it's marketed for.

Bilberry fruit extract monograph & interactionsBilberry Extract by Herbadiet Drug Interactions
HelloPharmacist Interaction Report
Herbadiet Bilberry Extract contains one active ingredient—bilberry fruit extract—which interacts with several medication types.
The most serious concern is with blood thinners and antiplatelet drugs (like warfarin, aspirin, or clopidogrel): bilberry may increase bleeding risk because research shows its compounds can slow blood clotting. This is rated Moderate severity.
Read the full breakdown — every affected drug type, severity by severity
Bilberry also has Moderate interactions with diabetes medications. It may lower blood sugar on its own, so combining it with drugs that do the same could push your glucose too low.
Additionally, animal research suggests bilberry might speed up how your liver breaks down certain medications processed through a specific enzyme (CYP2E1), potentially making those drugs less effective—though this effect wasn't consistent across all studies. Erlotinib (Tarceva), a cancer drug, may also be less effective if you take bilberry at the same time, based on lab research.
Altogether, these interactions span 275 individual medications. Use the search tool below to check your exact prescriptions before starting this supplement.

For each ingredient in Bilberry Extract with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Bilberry fruit extract
Anticoagulant/Antiplatelet Drugs
Theoretically, bilberry fruit extract might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro, animal, and clinical research suggest that anthocyanidin extracts from bilberry can inhibit platelet aggregation.
Antidiabetes Drugs
Theoretically, bilberry leaf or fruit extract may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that bilberry leaf extract might have blood glucose-lowering activity. Also, one small clinical trial in patients with type 2 diabetes shows that taking bilberry fruit extract 470 mg as a single dose prior to an oral glucose tolerance test lowers plasma glucose levels when compared with placebo.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Theoretically, bilberry fruit extract might decrease levels of drugs metabolized by CYP2E1.
Animal research shows that exposure to small concentrations of bilberry extract in drinking water for around one month increased CYP2E1 activity by 31%. However, exposure over a 2-month period did not increase CYP2E1 activity. This effect has not been reported in humans.
Erlotinib (Tarceva)
Theoretically, bilberry fruit extract might reduce the efficacy of erlotinib.
In vitro research suggests that bilberry fruit extract and its constituents, delphinidin and delphinidin-3-O-glucoside, inhibit the activity of erlotinib. This interaction has not been reported in humans.
