Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
This product contains one active ingredient: boneset aerial parts extract (the stems, leaves, and flowers of the boneset plant in dried, concentrated form). The liquid also includes two inactive ingredients — glycerin and water — to deliver the extract.
Does it work?
Not established
The evidence we hold shows insufficient reliable evidence to rate boneset for the common cold. That means the studies available aren't solid enough for us to say whether it works or doesn't.
If you're considering it for another reason, that data isn't on file here.
How safe is it?
Well-documented data
Boneset has limited safety data, and the plant contains alkaloids that *may* be toxic to the liver — though the exact risk hasn't been fully pinned down. The facts advise caution and short-term use only, if at all.
Orally, boneset is reported to cause nausea, severe diarrhea, and vomiting in some people. At higher doses, liver toxicity and liver damage are possible.
Boneset is rated possibly unsafe in pregnancy and should be avoided due to potential toxicity and lack of safety data. It should also be avoided while breastfeeding because safety hasn't been established and harmful alkaloids may pass to your infant.
Meds to double-check
Moderate interaction found
If you take CYP3A4 inducer medications (drugs that speed up liver processing), including carbamazepine and others, talk with your doctor or pharmacist before starting boneset. The interaction is Moderate severity and centers on how these drugs may increase the liver's conversion of boneset's alkaloids into potentially toxic forms.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
Boneset Alcohol Free is a single-ingredient liquid extract with very limited evidence for the common cold and documented liver-toxicity concerns. It's not recommended during pregnancy or breastfeeding.
Before starting it — especially if you take seizure medications, heart drugs, or anything metabolized by your liver — check your exact medications with your own doctor or pharmacist.

Boneset Alcohol Free by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Boneset Alcohol Free by Hawaii Pharm contains one active ingredient — boneset aerial parts extract — which has documented interactions with medications.
The most serious concern involves CYP3A4 inducers (Moderate severity), a group of drugs that speed up how your liver breaks down certain substances. Boneset contains plant alkaloids that your liver processes through the same pathway; if a CYP3A4 inducer drug is present, it may increase how quickly these alkaloids are converted to potentially toxic forms.
Carbamazepine (Tegretol), used for seizures and nerve pain, is one example listed.
Read the full breakdown — every affected drug type, severity by severity
Because boneset's alkaloid content hasn't been fully characterized, we can't predict all the downstream effects, but the concern centers on liver toxicity and how other drugs might amplify that risk.
Altogether, these interactions span 88 individual medications. Before you start boneset, check your exact medications with the search tool on this page.

Boneset Aerial Parts Extract, Dry
Cytochrome P450 3A4 (Cyp3A4) Inducers
Boneset belongs to the genus Eupatorium, and many species of this genus contain hepatotoxic pyrrolizidine alkaloids (PAs). Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.
