Bellafoline, Phenobarbital falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Boneset interacts with.

Boneset belongs to the genus Eupatorium, and many species of this genus contain hepatotoxic pyrrolizidine alkaloids (PAs). Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the important twist with this pairing. The concern is not about how boneset changes your phenobarbital. It's the other way around. The phenobarbital in Belladenal is a known booster (an inducer) of a liver enzyme called CYP3A4. Boneset can contain pyrrolizidine alkaloids, plant compounds that this same enzyme turns into liver-toxic byproducts. So in theory, taking phenobarbital could rev up that conversion and make boneset rougher on your liver.

I'll be honest, this is a theoretical concern based on how these things work, not on real patient studies, so don't panic. But boneset is already a bit risky for the liver on its own. My advice is simply to run it by your pharmacist or doctor before combining them.
