Sotorasib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Boneset interacts with.

Boneset belongs to the genus Eupatorium, and many species of this genus contain hepatotoxic pyrrolizidine alkaloids (PAs). Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the key thing to understand about pairing boneset with sotorasib (Lumakras): the concern isn't about your cancer medicine getting weaker or stronger. It's actually the other way around. Boneset can contain pyrrolizidine alkaloids, plant compounds that may be hard on the liver. Sotorasib speeds up a liver enzyme called CYP3A4, which is the same enzyme that turns those alkaloids into more toxic byproducts. So in theory, the combination could mean more liver stress.

I want to be honest that this is a theoretical worry based on how these compounds behave, not on real patient studies. Still, sotorasib already carries its own risk of liver irritation, so stacking another liver burden isn't ideal. Please don't change anything on your own, but do tell your oncologist or pharmacist before using boneset.

What Is Sotorasib Used For?

Brand name: Lumakras

Lumakras is used to treat adults with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have already received at least one prior cancer treatment. This use was approved based on tumor response data, and continued approval depends on results from ongoing confirmatory studies.

Lumakras is also approved — used together with panitumumab — for adults with KRAS G12C-mutated metastatic colorectal cancer (mCRC) who have already received prior chemotherapy regimens containing fluoropyrimidine, oxaliplatin, and irinotecan. In both cases, patients must have a confirmed KRAS G12C mutation detected by an FDA-approved test before starting treatment.
