Nafcillin Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Boneset interacts with.

Boneset belongs to the genus Eupatorium, and many species of this genus contain hepatotoxic pyrrolizidine alkaloids (PAs). Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the gist with nafcillin and boneset. Nafcillin isn't the problem child here, the herb is. Boneset can contain compounds called pyrrolizidine alkaloids that may stress the liver. Nafcillin happens to rev up a liver enzyme (CYP3A4) that turns those plant compounds into more harmful byproducts. So in theory, taking them together could nudge up the risk of liver irritation.

I'll be honest, this is a theoretical concern based on how the drug and herb behave, not on real patient studies. Nafcillin itself can occasionally bother the liver too, so stacking another liver stressor isn't ideal. I'd skip boneset supplements while on nafcillin and check with your pharmacist or doctor first. Don't change your antibiotic on your own.
