Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Boneset interacts with.

Boneset belongs to the genus Eupatorium, and many species of this genus contain hepatotoxic pyrrolizidine alkaloids (PAs). Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here is the key thing to understand about this pairing. The concern is not that boneset changes how well Fosaprepitant (Emend) works. Instead, it runs the other way. Boneset contains pyrrolizidine alkaloids, which are plant compounds that can be hard on the liver. Your body uses an enzyme called CYP3A4 to handle those compounds, and turning that enzyme up can actually create more of the toxic byproducts.

Fosaprepitant can give the CYP3A4 enzyme a short-term nudge, so in theory it could make boneset's liver burden worse. I want to stress this is theoretical, based on how these substances behave, not on real patient reports. Still, since Emend is usually given during cancer chemo when your liver is already working hard, I would skip boneset and check with your oncology team or pharmacist first.
