Belladonna, Phenobarbital falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Boneset interacts with.

Boneset belongs to the genus Eupatorium, and many species of this genus contain hepatotoxic pyrrolizidine alkaloids (PAs). Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the twist with this pairing. We usually worry that a drug will change how an herb is handled, but here it's the other way around. The phenobarbital in your Bellophen revs up a liver enzyme called CYP3A4. Boneset comes from a plant family that can contain pyrrolizidine alkaloids, and that same enzyme can turn those compounds into byproducts that are harder on the liver. So in theory, taking them together could nudge up the risk of liver irritation.

I want to be honest about the evidence: this is theoretical, based on how these substances behave, not real-world cases. Still, both boneset and a barbiturate can stress the liver on their own. Given that, plus phenobarbital's sedation, I'd run this by your pharmacist or doctor before combining them.
