Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Boneset interacts with.

Boneset belongs to the genus Eupatorium, and many species of this genus contain hepatotoxic pyrrolizidine alkaloids (PAs). Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the thing to keep straight with this pairing: it's not really about boneset changing how your Lorbrena works. Lorlatinib actually speeds up an enzyme in your liver called CYP3A4. Boneset's worry is that it contains plant compounds (pyrrolizidine alkaloids) that this same enzyme can turn into substances that may be hard on the liver. So in theory, taking them together could nudge more of boneset's compounds toward those liver-stressing byproducts.

I'll be honest, this is a theoretical concern based on how the chemistry lines up, not on real-world cases. But lorlatinib already asks a lot of your liver, and your team likely checks your liver labs. I'd skip boneset and run it by your oncologist or pharmacist first.

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
