Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Carnosine Supreme contains two active ingredients. Benfotiamine is a form of vitamin B1 (thiamine) that your body absorbs more readily than standard thiamine.
Carnosine is a naturally occurring compound made from two amino acids (alanine and histidine) that your muscle and nerve cells produce on their own. The product also includes microcrystalline cellulose and vegetable stearate as inactive ingredients to hold the capsule together.
Does it work?
Moderate evidence
The evidence for this product's ingredients is mixed and limited. Benfotiamine may help with diabetic nerve pain (neuropathy) but appears ineffective for diabetic kidney damage (nephropathy).
For carnosine, we don't have enough reliable evidence to say whether it works for age-related cognitive decline, Alzheimer disease, athletic performance, cataracts, or general cognitive impairment — the research just isn't established yet in our data.
How safe is it?
Well-documented data
Both ingredients are generally well tolerated in short-term studies, but long-term safety data are limited. With benfotiamine, a small number of trial participants reported allergic skin reactions or mild stomach discomfort, though these rates matched placebo.
Carnosine can cause decreased appetite, dry mouth, minor weight changes, foot and joint pain, and in rare cases pneumonia has been reported. For pregnancy and breastfeeding: the safety data advises against using carnosine — there isn't enough information to know it's safe.
Benfotiamine safety during breastfeeding hasn't been well studied, so caution is advised. Talk with your doctor or pharmacist about either ingredient if you're pregnant or nursing.
Meds to double-check
Moderate interaction found
If you take antidiabetes drugs — like metformin, insulin, sulfonylureas, or GLP-1 agonists — check with your doctor or pharmacist before adding this product. Carnosine may increase your low blood sugar risk, and you may need dose adjustments or closer monitoring.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
Carnosine Supreme might interest you if you're exploring options for nerve pain or cognitive support, but the evidence for most uses is still uncertain. If you take any diabetes medications, you'll need to check those carefully with your doctor or pharmacist first — carnosine may lower your blood sugar further.
Pregnant or breastfeeding? The safety data on these ingredients don't support use, so discuss it with your healthcare provider before starting.
