Carob is possibly effective for diarrhea — that's the main use with some evidence behind it. For athletic performance, diabetes, and familial hypercholesterolemia (a genetic cholesterol disorder), the evidence we hold isn't established enough to rate it.

Carob is widely eaten as food and is generally well tolerated in the amounts you'd find in normal diet. The caution here is that concentrated supplement doses — the kind you get from a liquid extract — haven't been well studied.
Use caution, especially at higher doses. There are case reports of allergic reactions in infants, including rash and vomiting, and very rarely, serious intestinal problems in premature infants given carob-thickened formula.
These reports don't mean carob is unsafe for adults at normal doses, but they show why you want to be careful with dosing and why infants need their doctor's close watch.
