Andrographis is generally well tolerated over the short term, though long-term safety hasn't been firmly established. The most common side effects are stomach discomfort, altered taste, diarrhea, dizziness, fatigue, headache, nausea, vomiting, itching, rash, and hives—especially at higher doses or beyond two weeks of use.

Serious allergic reactions, including anaphylaxis, are rare but reported. Skin reactions, including severe ones like Stevens-Johnson syndrome, have also been documented.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
