D.L. Duo-Dophilus contains three active ingredients.
The first is Lactobacillus acidophilus (strain La-14), a beneficial bacterium found naturally in the gut and vagina. The second is Lactobacillus rhamnosus (strain Lr-32), another probiotic organism.
The third is FOS (fructooligosaccharides), which serves as a food source for these beneficial bacteria. The capsule itself is made from gelatin, microcrystalline cellulose (a binder), and vegetable stearate.
Does it work?
Moderate evidence
Lactobacillus acidophilus is rated Possibly Effective for four conditions: irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis. The evidence is promising but not yet conclusive for these uses.
For atopic disease (allergic and immune conditions), it is rated Possibly Ineffective. For acute respiratory distress syndrome (ARDS), the evidence we hold is insufficient to rate it.

How safe is it?
Well-documented data
Lactobacillus acidophilus is generally well tolerated in healthy adults and children. The most common gastrointestinal side effects — such as mild stomach discomfort, bloating, gas, or diarrhea — are uncommon.
Because this product contains live organisms, there is a theoretical risk of infection in certain patient populations (those with a weakened immune system, critical illness, or conditions that allow bacteria to cross the gut barrier), though such cases are rare. One clinical trial reported a small number of rash cases, though it's unclear whether they were linked to this strain of Lactobacillus or to other factors.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
