Malic acid in food amounts is generally well tolerated. However, the safety profile for supplement-level doses hasn't been well studied, so caution is advised if you're taking it this way.
When taken by mouth, side effects like diarrhea and nausea have been reported in some people taking a combination product that included malic acid, though it's unclear whether malic acid or the other ingredient (magnesium) was responsible. If you apply it to your skin, it can cause irritation, peeling, and in sensitive individuals, hives or contact dermatitis.
