Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: Dong Quai extract, a traditional herbal preparation. The liquid also includes certified organic cane alcohol and distilled water as inactive ingredients to preserve and deliver the extract.
Does it work?
Not established
The effectiveness data we hold on Dong Quai shows insufficient evidence to rate it for iron deficiency anemia, osteoporosis, heart disease, migraine headache, or eczema. In other words, we don't have reliable research establishing whether it works for any of these uses.
How safe is it?
Well-documented data
Orally, Dong Quai is generally well-tolerated, though some people report burping and flatulence. It can increase your sensitivity to sunlight, so take care in the sun if you use it.
The safety data advises against use during pregnancy because it may stimulate the uterus and affect bleeding. There isn't enough safety information to recommend it while breastfeeding — talk with your doctor or pharmacist about what's right for you.
The herb also contains compounds that are known to be carcinogenic, though it's unclear whether the amounts in this product pose a real cancer risk with long-term or high-dose use.
Meds to double-check
Major interaction found
Before taking Herb Pharm's Dong Quai, double-check any blood thinners (warfarin, heparin, apixaban, dabigatran, edoxaban, rivaroxaban) or antiplatelet drugs (aspirin, clopidogrel, ticlopidine) you take — the risk of bleeding is Major to Moderate. Also confirm any estrogen-based birth control or hormone replacement therapy with your doctor or pharmacist.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Major medication interactions have been identified, and safety information is well characterized.
Dong Quai is best reserved for people who aren't on blood thinners or hormone medications — and even then, the evidence for what it actually does is thin. If you take warfarin, other anticoagulants, antiplatelet drugs, or estrogen-based therapy, skip this product or check with your doctor or pharmacist first.
Pregnant people and those breastfeeding should avoid it.

Dong Quai by Herb Pharm Drug Interactions
HelloPharmacist Interaction Report
Herb Pharm's Dong Quai extract interacts with blood thinners and hormone medications through its coumarin and estrogenic content.
The most serious concern is with warfarin (Coumadin): case reports show that dong quai can strengthen warfarin's blood-thinning effect and raise your bleeding risk, even after just a few weeks at typical doses.
Read the full breakdown — every affected drug type, severity by severity
Dong quai also poses a moderate bleeding risk with other blood thinners and antiplatelet drugs — the kind you might take after a heart attack or stroke or for certain blood clots. Animal studies and some human reports suggest it can thin your blood further, though the research isn't entirely consistent.
Separately, dong quai may interfere with estrogen-based medications (like birth control or hormone replacement therapy) by competing for the same receptors in your body.
Before you start this product, check with your doctor or pharmacist about any blood thinners, aspirin-type drugs, or hormone medications you're on. Altogether, these interactions span 163 individual medications.

Dong Quai extract
Warfarin (Coumadin)
Dong quai may increase the risk of bleeding when used with warfarin.
Case reports suggest that concomitant use of dong quai with warfarin can increase the anticoagulant effects of warfarin and increase the risk of bleeding. In one case, after 4 weeks of taking dong quai 565 mg once or twice daily, the international normalized ratio (INR) increased to 4.9. The INR normalized 4 weeks after discontinuation of dong quai.
Anticoagulant/Antiplatelet Drugs
Theoretically, dong quai may increase the risk of bleeding when used with anticoagulant or antiplatelet drugs; however, research is conflicting.
Animal studies suggest that dong quai has antithrombin activity and inhibits platelet aggregation due to its coumarin components. Additionally, some case reports in humans suggest that dong quai can increase the anticoagulant effects of warfarin. However, clinical research in healthy adults shows that taking 1 gram of dong quai root daily for 3 weeks does not significantly inhibit platelet aggregation or cause bleeding. Until more is known, use dong quai with caution in patients taking antiplatelet/anticoagulant drugs.
Estrogens
Theoretically, dong quai may reduce the effects of estrogens.
Dong quai has estrogenic effects. Theoretically, concomitant use of large amounts of dong quai might interfere with hormone replacement therapy due to competition for estrogen receptors.
