Dong quai has estrogenic effects. Theoretically, concomitant use of large amounts of dong quai might interfere with hormone replacement therapy due to competition for estrogen receptors.

If you take estrogen (whether for menopause symptoms or as hormone replacement), here's the idea behind pairing it with Dong Quai. Some compounds in Dong Quai are thought to be weakly estrogen-like and may sit on the same receptors your estrogen medicine uses. In theory, large amounts could compete with your prescribed estrogen and slightly blunt how well it works, meaning symptoms like hot flashes, night sweats, or mood changes might not be controlled as well.

Honestly, this is a theoretical concern based only on lab studies, not solid human data, so I wouldn't lose sleep over it. Dong Quai can also affect bleeding, which is another reason to check first. Please run it by your pharmacist or doctor before starting the herb.
