Estradiol falls under ESTROGENS — the group Dong Quai interacts with.

Theoretically, dong quai may reduce the effects of estrogens.
Dong quai has estrogenic effects. Theoretically, concomitant use of large amounts of dong quai might interfere with hormone replacement therapy due to competition for estrogen receptors.

Here's the idea behind this one. Some compounds in Dong Quai are thought to act a little like estrogen and may compete for the same estrogen receptors your estradiol is trying to work on. In theory, that competition could blunt how well your estrogen therapy works, so symptoms like vaginal dryness, irritation, painful intercourse, or hot flashes might not be controlled as well.

I want to be honest about the evidence, though. This is theoretical and comes mostly from lab studies, not real patient outcomes, and it would likely take large supplement doses rather than occasional use to matter. Estradiol isn't a prodrug, so nothing here changes how your body activates it. Before adding Dong Quai, just check with your pharmacist or doctor, especially since this herb can also affect bleeding.
