If you're taking esterified estrogens (Menest or Estratab) for hot flashes or other menopause symptoms, here's the idea behind this pairing. Dong Quai contains plant compounds that some researchers thought could act a little like estrogen and bump up against the same estrogen receptors your medication uses. In theory, large amounts of the herb might compete with your estrogen and slightly blunt how well it works, so you could notice symptoms like hot flashes or vaginal dryness creeping back.

I want to be honest about the strength of this one: it comes from lab and animal research only, and studies haven't clearly shown Dong Quai behaves like estrogen in real people. So this is more of a theoretical caution than a proven problem. Still, before adding a concentrated Dong Quai supplement, run it by your pharmacist or doctor first.
