Elderberry Plus contains 5 active ingredients. You've got chaga and reishi mushroom, two fungi traditionally used in herbal medicine.
The two elderberry components—Elderberry Fruit Extract and Elderberry Fruit Juice Concentrate—come from the same plant but in different forms. Turkey Tail is also included, though we hold no interaction data for it.
The product also contains inactive ingredients: cane sugar, purified water, glycerin, and citric acid. These are fillers and preservatives that help make the liquid stable and palatable.
Does it work?
Not established
The evidence for what this product actually does is pretty thin across the board. For chaga, we don't have reliable evidence it helps with heart disease, diabetes, or COVID-19.
Elderberry hasn't been reliably shown to help with hay fever, heart disease, fatigue, lung disease, thinking, or respiratory infections. Reishi doesn't appear effective for high cholesterol, and we don't have good evidence for altitude sickness, Alzheimer's, or athletic performance either.
The data we hold simply doesn't establish these ingredients work for their common uses.
How safe is it?
Well-documented data
The elderberry extracts in this product, when made from ripe fruit and properly prepared, are generally well tolerated. However, the raw or unripe berries, seeds, leaves, and bark can be toxic and cause nausea, vomiting, severe diarrhea, weakness, dizziness, and other symptoms—cooking eliminates that risk, and this liquid should be safe on that front.
A rare case of autoimmune hepatitis has been reported with long-term elderberry use in someone with underlying autoimmune disease. Chaga has limited human safety data.
High consumption carries a risk of kidney problems because chaga contains high oxalate levels; two cases of kidney failure and one of kidney injury have been reported with excessive use. Reishi is generally well tolerated short-term, though long-term safety isn't well studied.
Common mild side effects include dizziness, dry mouth, itching, nausea, rash, and stomach upset. Respiratory allergy to reishi spores, sore throat, and runny nose can occur, and rare cases of insomnia and headache have been reported.

Before you take Elderberry Plus, double-check with your pharmacist if you're on blood thinners like warfarin or aspirin (chaga and reishi can increase bleeding risk), medications that lower blood pressure (reishi adds to that effect), diabetes drugs (chaga and reishi may lower blood sugar further), drugs that suppress your immune system after transplant or for autoimmune disease (both elderberry extracts and chaga may work against them), or pazopanib for cancer (a case report links it to serious liver injury). No interactions are documented for Turkey Tail, as we could not check that ingredient.

The most serious concern involves chaga and blood thinners (anticoagulants and antiplatelet drugs): chaga may theoretically increase bleeding risk by slowing how your platelets clump together, though this hasn't been confirmed in humans yet—this is rated Moderate severity.

You'll also want to watch for interactions with immunosuppressants (used after transplants or for certain autoimmune conditions). Both the elderberry extracts and chaga may theoretically interfere with these medications by ramping up your immune system.
Reishi mushroom adds another concern: it can lower blood pressure, so combining it with blood pressure medications might drop your pressure too far. Reishi may also theoretically increase bleeding risk at higher doses.
Both chaga and reishi could theoretically lower blood sugar too much if you're on diabetes medications.
One more thing: the elderberry extracts in this product may theoretically interact with pazopanib, a cancer drug—there's a case report of severe liver injury in a patient taking both. Altogether, these interactions span 487 individual medications.

Reishi
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
Chaga
Anticoagulant/Antiplatelet Drugs
Theoretically, chaga may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that chaga extract can inhibit platelet aggregation. This effect has not been reported in humans.
Antidiabetes Drugs
Theoretically, taking chaga with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research suggests that chaga might decrease blood glucose levels and increase insulin levels. This has not been reported in humans.
Immunosuppressants
Theoretically, chaga might interfere with immunosuppressive therapy.
In vitro research suggests that certain constituents of chaga stimulate immune function. This has not been reported in humans.
Elderberry Fruit Extract
Immunosuppressants
Theoretically, elderberry might interfere with immunosuppressant therapy due to its immunostimulant activity.
Elderberry has immunostimulant activity, increasing the production of cytokines, including interleukin and tumor necrosis factor.
Pazopanib (Votrient)
Theoretically, elderberry might interact with pazopanib, potentially increasing the risk of adverse effects.
