The evidence for elderberry is limited. The data we hold shows insufficient reliable evidence to rate its use for hay fever, cardiovascular disease, chronic fatigue syndrome, COPD, cognitive function, or respiratory tract infections.
That doesn't mean it doesn't work — it means the studies aren't yet solid enough to make a confident claim either way.
How safe is it?
Well-documented data
Properly cooked elderberry syrup is generally well tolerated. The risk comes from raw or unripe berries, seeds, leaves, and bark, which contain cyanogenic glycosides (compounds that break down into cyanide) and can cause nausea, vomiting, severe diarrhea, weakness, dizziness, numbness, and even stupor — but cooking eliminates this toxin, which is why a prepared syrup should be safe on that front.
One case report noted autoimmune hepatitis in a person with underlying autoimmune disease who took elderberry for years, though this is rare. If you're pregnant or breastfeeding, there isn't enough safety data, so it's best to avoid elderberry supplements until you've talked it over with your doctor or pharmacist.
