Does it work?
Moderate evidence
The evidence for what these ingredients actually do is limited. Tribulus and tongkat ali are possibly effective for sexual desire, but ginseng's main support is for upper respiratory infections—not athletic performance.
Maca, ginseng for athletic performance, horny goat weed for sexual dysfunction, butea superba for erectile dysfunction, and several other uses have insufficient reliable evidence or possibly ineffective ratings. The data we hold doesn't establish that this combination will boost endurance.
How safe is it?

Maca and tribulus are generally well tolerated as foods, but long-term safety as supplements hasn't been well studied for either. Tribulus and ginseng may cause headache; tribulus can trigger stomach pain, nausea, vomiting, or diarrhea, though rates were similar to placebo in one study.
Horny goat weed may cause dizziness, dry mouth, nosebleed, or vomiting with short-term use. All five checked ingredients carry warnings against use during pregnancy and breastfeeding because safety data are lacking or suggest possible harm.

Beyond that, you'll want to check if you take blood pressure medications, diabetes drugs, or immune-suppressing medications. Tribulus terrestris and horny goat weed may lower blood pressure further (raising the risk of dizziness or fainting); ginseng and tribulus may also lower blood sugar, increasing the risk of low blood sugar (hypoglycemia).
Ginseng might interfere with immunosuppressants. Horny goat weed could theoretically boost the effects of estrogens and increase bleeding risk if you're on blood thinners or antiplatelet drugs like aspirin.
Tongkat Ali extract may reduce the effectiveness of propranolol (a beta-blocker for heart rate and blood pressure) and may theoretically raise levels of certain other medications your liver processes. Tribulus could raise lithium levels if you take it for mood disorders.
Butea superba theoretically might dangerously lower blood pressure if combined with erectile dysfunction medications like sildenafil (Viagra).

Epimedium Root Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, horny goat weed might increase the risk of bleeding.
In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.
Antihypertensive Drugs
Theoretically, horny goat weed might increase the risk of hypotension.
Laboratory research suggests that horny goat weed might have hypotensive effects. This effect has not been reported in humans.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Cytochrome P450 2B6 (Cyp2B6) Substrates
Theoretically, horny goat weed might increase the effects and side effects of CYP2B6 substrates.
In vitro, horny goat weed leaf extract inhibits CYP2B6. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Estrogens
Theoretically, concomitant use of horny goat weed with estrogens might increase their therapeutic and adverse effects.
In vitro evidence suggests that horny goat weed has estrogenic activity. In clinical research, horny goat weed has been shown to increase blood levels of estrogen in some females.
Tribulus terrestris
Antidiabetes Drugs
Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.
Antihypertensive Drugs
Theoretically, taking tribulus with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that tribulus can lower blood pressure by inhibiting angiotensin-converting enzyme (ACE). Tribulus has also demonstrated hypotensive effects in pre-hypertensive adults.
Lithium
Theoretically, tribulus might increase the levels and clinical effects of lithium.
Tribulus is thought to have diuretic properties. Due to these potential diuretic effects, tribulus might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
Tongkat Ali Extract
Propranolol (Inderal)
Eurycoma longifolia can reduce the levels and clinical effects of propranolol.
A small clinical study in healthy persons shows that taking a single dose of a water-based Eurycoma longifolia extract 200 mg, in combination with a single dose of propranolol 80 mg, reduces the propranolol area under the curve (AUC) by 29%, reduces the peak concentration by 42%, and increases time to peak concentration by 86% when compared with control. Since the elimination half-life of propranolol did not change, it seems that Eurycoma longifolia alters the kinetics of propranolol by decreasing its absorption in the gut, and not by altering its metabolism. It is not known if separating administration will prevent this interaction.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, Eurycoma longifolia might increase levels CYP1A2 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.
Cytochrome P450 2A6 (Cyp2A6) Substrates
Theoretically, Eurycoma longifolia might increase levels of CYP2A6 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2A6 enzymes. This effect has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.
Testosterone
Theoretically, Eurycoma longifolia may further increase levels of testosterone.
A clinical study in aging males with testosterone levels below 300 ng/dL shows that taking a specific water extract of Eurycoma longifolia roots (Physta; Biotropics Malaysia) 100-200 mg daily with breakfast for 12 weeks increases total testosterone levels by 8% to 11% when compared with placebo. It is unclear whether this increase would occur in individuals with normal testosterone levels.
Ginseng root extract
Warfarin (Coumadin)
American ginseng seems to decrease the effectiveness of warfarin therapy.
Healthy patients receiving warfarin 5 mg daily, who also take American ginseng 1 gram twice daily, seem to have a significantly reduced international normalized ratio (INR).
Antidiabetes Drugs
Theoretically, taking American ginseng with antidiabetes drugs might increase the risk of hypoglycemia.
American ginseng seems to lower postprandial blood glucose. Theoretically, concomitant use with antidiabetes drugs might enhance blood glucose lowering effects and possibly cause hypoglycemia.
Immunosuppressants
Theoretically, American ginseng use might interfere with immunosuppressive therapy.
American ginseng seems to stimulate immune function. Theoretically, American ginseng might decrease the effectiveness of immunosuppressant drugs.
Monoamine Oxidase Inhibitors (Maois)
Theoretically, American ginseng can interfere with MAOI therapy.
There is one case report of insomnia, headache, and tremors when an unspecified ginseng product was used with phenelzine (Nardil), an MAOI. There is also one case report of hypomania when an unspecified ginseng product was used with phenelzine. Theoretically, American ginseng may interfere with MAOI therapy.
Butea superba
Phosphodiesterase-5 Inhibitors
Butea superba is thought to inhibit phosphodiesterase and cause vasodilation. Theoretically, concurrent use of phosphodiesterase-5 inhibitors and Butea superba might result in additive vasodilation and hypotension. Phosphodiesterase-5 inhibitors include sildenafil (Viagra), tadalafil (Cialis), and vardenafil (Levitra).
