The active digestive enzymes include bromelain (from pineapple), papain (from papaya), protease enzymes (three types), amylase (breaks down starches), lipase (breaks down fats), lactase (breaks down milk sugar), hemicellulase, and cellulase.
The probiotic strains are Bifidobacterium animalis lactis, B. bifidum, Lactobacillus acidophilus, L. bulgaricus, and L. brevis. It also contains grapefruit pectin and psyllium for fiber, plus oat bran.
The inactive ingredients are vegetable cellulose, silica, rice bran, and vegetable magnesium stearate.
Does it work?

The probiotics show some promise: B. bifidum and L. acidophilus are both possibly effective for irritable bowel syndrome and respiratory tract infections. L. acidophilus is also possibly effective for H. pylori infection, antibiotic-related diarrhea, and bacterial vaginosis.
Psyllium is effective for constipation and likely effective for high cholesterol and heart disease. Lactase is effective for lactose intolerance.
However, for most other uses listed — bromelain for hay fever or burns, papain for muscle soreness or herpes, and the other conditions — there isn't enough reliable evidence to rate their effectiveness.
How safe is it?
Well-documented data
Bromelain is generally well tolerated short-term, but long-term safety data are limited. Common side effects include diarrhea, flatulence, stomach upset, and headache.
It can trigger allergic reactions in sensitive people, including swelling or difficulty swallowing if inhaled. The probiotics are generally well tolerated in healthy people but should be used cautiously by anyone with a weakened immune system.
Possible side effects include bloating, flatulence, and mild abdominal discomfort. Papain is well tolerated as food but may irritate the digestive tract and cause allergic reactions; high doses carry rare risks of esophageal perforation and severe gastritis.
Psyllium is generally well tolerated when taken with adequate water but may cause abdominal pain, bloating, diarrhea, or constipation; rare serious effects include bowel or esophageal obstruction, and allergic reactions can occur with repeated use. Protease enzymes may cause digestive upset or allergic reactions.

Several drug types are affected moderately by this product. Bromelain may increase bleeding risk with blood thinners and antiplatelet drugs like warfarin and aspirin.
Papain can boost warfarin's effects unpredictably. The three probiotic strains we checked (B. bifidum, L. acidophilus, and L. bulgaricus) may lose effectiveness if taken at the same time as antibiotics, since antibiotics can kill the live organisms.
Pectin may reduce absorption of cholesterol-lowering drugs (lovastatin), certain antibiotics (tetracyclines), and the heart medication digoxin — potentially weakening them. Psyllium can also interfere with several medications: it may reduce lithium levels (used for mood disorders), lower the seizure-prevention effect of carbamazepine, decrease olanzapine (an antipsychotic) effectiveness, and may affect birth control pills and digoxin.
Bromelain also has a minor interaction with tetracycline antibiotics, raising theoretical absorption concerns, though one study found no practical effect. Psyllium may variably affect other oral medications and has a minor interaction with digoxin, though some evidence suggests psyllium doesn't actually impact digoxin absorption.

Psyllium
Carbamazepine (Tegretol)
Theoretically, blond psyllium might reduce the effects of carbamazepine and increase the risk for convulsions.
Blond psyllium might reduce carbamazepine absorption and serum levels. A preliminary study using blond psyllium reported decreased carbamazepine bioavailability due to binding of the drug to psyllium, as well as reduction of available fluid in the gut for dissolution of the drug.
Lithium
Theoretically, taking blond psyllium at the same time as lithium might reduce lithium absorption.
The fiber in blond psyllium might decrease the absorption of lithium. Some case reports describe a reduction in plasma lithium levels with concomitant administration of blond psyllium. This was reversed when psyllium was stopped.
Metformin (Glucophage)
Theoretically, blond psyllium might increase the therapeutic and adverse effects of metformin.
Concurrent use of blond psyllium with metformin slows and increases metformin absorption. To avoid changes in absorption, take psyllium 30-60 minutes after metformin.
Olanzapine (Zyprexa)
Theoretically, taking blond psyllium at the same time as olanzapine might reduce olanzapine absorption.
The fiber in blond psyllium might decrease the absorption of olanzapine. A single case report describes a reduction in the effectiveness of olanzapine when it was taken concomitantly with an unspecified type of psyllium 3 grams orally twice daily. This effect was reversed when psyllium was stopped.
Digoxin (Lanoxin)
Theoretically, taking blond psyllium at the same time as digoxin might reduce digoxin absorption.
Psyllium might bind digoxin in the gut. However, some clinical evidence suggests that psyllium does not impact digoxin absorption.
Ethinyl Estradiol
Theoretically, taking blond psyllium at the same time as ethinyl estradiol might alter levels of estradiol.
Concurrent use of blond psyllium with ethinyl estradiol results in a slight increase in the extent of ethinyl estradiol absorption and a slower rate of absorption. However, this is unlikely to be clinically significant.
Oral Drugs
Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.
B. bifidum
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
Lactobacillus acidophilus
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
L. bulgaricus
Antibiotic Drugs
Theoretically, taking Lactobacillus delbrueckii with antibiotic drugs might decrease the effectiveness of L. delbrueckii.
Lactobacillus delbrueckii preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. delbrueckii preparations by at least two hours.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Grapefruit Pectin
Digoxin (Lanoxin)
Theoretically, pectin might reduce the absorption of digoxin, potentially decreasing its effectiveness.
A small clinical study shows that taking digoxin with a kaolin-pectin suspension reduces the absorption of digoxin by about 62%. It is unclear if these effects are due to pectin, kaolin, or the combination.
Lovastatin (Mevacor)
Theoretically, pectin might reduce the absorption of lovastatin, potentially decreasing its effectiveness.
Case reports suggest that concomitant use of pectin and lovastatin might reduce the cholesterol-lowering effect of lovastatin, possibly due to reduced intestinal absorption of lovastatin.
Tetracycline Antibiotics
Theoretically, pectin might reduce the absorption of tetracycline antibiotics, potentially decreasing their effectiveness.
A small clinical study shows that taking tetracycline with bismuth subsalicylate in a kaolin-pectin suspension reduces the absorption of tetracycline by about 34%. It is unclear if these effects are due to pectin, kaolin, bismuth subsalicylate, or the combination.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
