METFORMIN (Glucophage)

Your Kazano tablet combines two diabetes medicines: alogliptin and metformin. The piece that may interact with blond psyllium is the metformin. Because psyllium turns into a thick gel in your gut, it can slow down and slightly boost how much metformin gets absorbed. In theory, that could mean a bit more blood-sugar-lowering effect, along with metformin's usual gut side effects like nausea, an upset stomach, or loose stools.
Keep in mind this comes from lab-type research, not strong human studies, so the real-world impact is likely small. A simple fix many pharmacists suggest is to space them out, taking your Kazano first and the psyllium about 30 to 60 minutes later. Please run your exact timing by your pharmacist before changing anything.

Alogliptin, Metformin falls under ORAL DRUGS — the group Blond Psyllium interacts with.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the simple version when it comes to your Kazano (alogliptin and metformin) and psyllium fiber. Psyllium forms a thick gel in your gut, and that gel can change how fast and how much of a swallowed medicine gets absorbed. For most drugs this effect is small, but animal studies have specifically shown psyllium can slow down and slightly increase the absorption of the metformin part of your combo. The alogliptin side hasn't shown much concern.
Neither of these is a prodrug, so we're really just talking about timing, not a major safety issue. The easy fix is to take your Kazano first, then take your psyllium about 30 to 60 minutes later. If you ever notice unusual stomach upset or low blood sugar symptoms, check in with your pharmacist or doctor.
