These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Blond Psyllium interacts with whole categories of medication. Metformin, Sitagliptin falls into 2 of those categories, so it can interact with Blond Psyllium in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
METFORMIN (Glucophage)
1 of 2Concurrent use of blond psyllium with metformin slows and increases metformin absorption. To avoid changes in absorption, take psyllium 30-60 minutes after metformin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your Janumet combines two diabetes medicines: metformin and sitagliptin. The piece that overlaps with blond psyllium here is the metformin. Because psyllium forms a thick gel in your gut, it can grab onto metformin and slow how it gets absorbed, and in some lab work it actually seemed to increase the total amount that made it into the body. In theory, that could nudge metformin's effects up a bit, meaning more blood-sugar lowering and possibly more of its usual stomach complaints like nausea, bloating, or loose stools.
Keep in mind this is based only on early lab and animal data, so it is more of a caution than a proven problem. A simple fix many pharmacists suggest is spacing them out: take your psyllium about 30 to 60 minutes after your Janumet. Run this by your own pharmacist or doctor before changing anything.

ORAL DRUGS
2 of 2Why this applies to Metformin, Sitagliptin
Metformin, Sitagliptin falls under ORAL DRUGS — the group Blond Psyllium interacts with.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the practical picture with Janumet (which combines metformin and sitagliptin) and a fiber like blond psyllium. Because psyllium swells into a thick gel in your gut, it can change how quickly a pill gets absorbed if you swallow them at the same time. For metformin specifically, animal research actually suggests psyllium may slow down and slightly increase how much is absorbed. This is a minor interaction based on limited evidence, so it's not something to lose sleep over.
To keep things smooth, a simple habit helps: take your Janumet first, then wait about 30 to 60 minutes before your psyllium. Watch for any unusual low blood sugar signs (shakiness, sweating) or extra stomach upset, and check in with your pharmacist or doctor before changing anything.
