Hydrochlorothiazide falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the simple version: psyllium is a thick, gel-forming fiber, and when it's sitting in your gut at the same time as a pill, it can sometimes grab onto the medicine and change how much of it your body soaks up. With hydrochlorothiazide, this is mostly a theoretical concern, and the evidence is pretty thin. The worry would be slightly less of your water pill getting absorbed, which could mean a bit less blood pressure or fluid control.

The easy fix is timing. Take your hydrochlorothiazide first, then wait about 30 to 60 minutes before your psyllium. That spacing usually keeps the fiber from getting in the way. If you notice your blood pressure creeping up or more swelling than usual, check in with your pharmacist or doctor.

Blond psyllium is a soluble fiber from the seed husks of Plantago ovata, and it is one of the better-studied fiber supplements. It can help with both constipation and diarrhea and may modestly lower cholesterol and blood sugar. It is generally well tolerated when taken with plenty of water, but always start low and check with your pharmacist or doctor first.

What Is Hydrochlorothiazide Used For?

Brand names: Esidrix, Hydro, Microzide, HydroDiuril, Hydro-D, Oretic, Aquazide H, Aquazide, HCTZ

Hydrochlorothiazide treats high blood pressure (hypertension) in adults and children, either alone or combined with other blood pressure medicines. Lowering blood pressure reduces the risk of strokes and heart attacks.

It also treats edema (fluid buildup) linked to congestive heart failure, hepatic cirrhosis (liver scarring), and kidney conditions including nephrotic syndrome. Hydrochlorothiazide tablets are additionally indicated for edema caused by corticosteroid or estrogen therapy, and edema from acute glomerulonephritis or chronic renal failure. Inzirqo (the oral suspension) is specifically approved for both hypertension and edema in adult and pediatric patients.
