Here's the gist: when you take blond psyllium right alongside your metformin, that thick fiber gel can change how your body soaks up the medication. The theory, based on lab-type studies, is that it may slow down and slightly boost how much metformin gets absorbed. That could mean a little more of metformin's usual effects, including lower blood sugar and the common stomach complaints like nausea, loose stools, or belly upset.

The good news is this is easy to manage. The simplest fix is timing: take your metformin first, then wait about 30 to 60 minutes before your psyllium. Keep an eye out for any low blood sugar signs if you're also on insulin or other diabetes meds. The evidence here is weak and mostly theoretical, so don't panic, but do run your routine by your pharmacist or doctor.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the practical picture with psyllium and your metformin. Because psyllium turns into a thick gel in your gut, it can change how some pills get absorbed. For metformin specifically, animal studies suggest psyllium may slow down and slightly increase how much you take in. Metformin isn't a prodrug, so any extra absorbed simply means a bit more drug on board.

Metformin is used to improve blood sugar (glucose) control in people with type 2 diabetes mellitus. It is always used together with diet and exercise, not instead of them.

Metformin hydrochloride tablets and oral solution are approved for adults and children 10 years and older. Metformin hydrochloride extended-release tablets — including brands such as Fortamet, Glumetza, and Metformin Hydrochloride Extended Release — are approved for adults only.
