Rehmannia, poria mushroom, peony, and bupleurum are generally well tolerated based on traditional use, but human safety data are limited for all of them and product quality varies. Peony has been associated with gastrointestinal side effects including nausea, diarrhea, abdominal distension, and loss of appetite in some users—diarrhea occurred in about 5% of people taking a high dose for an extended period in one clinical trial.

Poria mushroom can rarely cause allergic reactions such as allergic rhinitis or asthma. Bupleurum-containing products have been linked to rare but serious lung and liver problems in combination formulas, though it's unclear whether bupleurum alone or other ingredients are responsible.
