Here's the gist with acarbose and bupleurum. Acarbose lowers your blood sugar by slowing how fast your meals turn into glucose. The concern is that some compounds in bupleurum, called saikosaponins, may push blood sugar in the opposite direction. So in theory, the herb could work against acarbose and make your numbers run higher than expected.

I want to be honest about how soft this evidence is. It comes from animal studies only, not from people, so it's more of a heads-up than a proven problem. If you do take both, the easy safeguard is checking your blood sugar a bit more often and watching for readings that creep up. Loop in your pharmacist or doctor before adding bupleurum so they can keep an eye on your diabetes control.
