Here's the gist of it: Sitagliptin (Januvia) works to bring your blood sugar down, but bupleurum may tug in the opposite direction. In animal studies, the saikosaponins in bupleurum actually nudged blood sugar up. So the worry is that taking bupleurum could partly blunt how well your Januvia controls your numbers, possibly leaving your readings higher than expected.

I want to be honest about the strength of this, though. It's theoretical, based on lab and animal research, not solid human studies, so it may not amount to much in real life. Sitagliptin isn't a prodrug, so this is about a competing effect on blood sugar, not how your body activates the drug. If you take both, just keep an eye on your home glucose checks and let your pharmacist or doctor know before adding bupleurum.

What Is Sitagliptin Used For?

Sitagliptin is used alongside diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus. It is available as tablets (Januvia, Zituvio, and generic sitagliptin products) and as an oral solution (Brynovin), all approved for this same use.

Sitagliptin is not for type 1 diabetes. It has not been studied in people with a history of pancreatitis, so its safety in that group is unknown.
