Bivalirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Bupleurum interacts with.

Theoretically, bupleurum might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro research suggests that saikosaponins, constituents of bupleurum, can inhibit platelet aggregation.

Here's the situation with these two. Bivalirudin is a strong blood thinner given through an IV, almost always in a hospital during procedures like angioplasty or stent placement. It directly blocks thrombin, a key clotting protein, so your blood is already much slower to clot while you're on it.

Bupleurum contains compounds called saikosaponins that, in lab dish studies only, slowed platelets (the cells that help form clots) from sticking together. Stacking a platelet-slowing herb on top of a powerful thinner could in theory raise bleeding risk, meaning easier bruising, bleeding gums, or harder-to-stop bleeding. That said, this is purely theoretical, based on test-tube data, not human studies. Since bivalirudin is used in a monitored hospital setting, this rarely comes up in practice. Still, always tell your care team about any supplements you take so they can keep an eye out.
