Bacillus subtilis is generally well tolerated in healthy adults, but the safety data is limited and quality can vary by product and strain. In clinical trials, one person reported a headache, and diarrhea was reported rarely.

There are also rare cases on record of serious infections (bacteremia and meningitis) linked to frequent consumption of natto (soybeans fermented with a related B. subtilis variety), though these occurred in people who were already immunocompromised—very elderly, diabetic, or dealing with kidney disease or cancer. Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
