Andrographis is generally well tolerated for short-term use, but long-term safety and product quality aren't well established. Mild side effects can include stomach discomfort, nausea, diarrhea, headache, fatigue, dizziness, altered taste, rash, and itching—especially at higher doses or beyond 14 days.

Rarely, severe allergic reactions including anaphylaxis have been reported. Skin reactions, including serious ones like Stevens-Johnson syndrome, have also been reported.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
