Innate Immune Support Ingredients & Drug Interactions
What is this page for?
First and foremost: checking Innate Immune Support against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

What’s inside
Full disclosure
This supplement contains 4 active ingredients: reishi fruiting body extract, astragalus root extract, andrographis (AP-Bio), and beta-glucan. Reishi is a mushroom used traditionally to support immune function; astragalus is a plant root with a long history in herbal medicine for similar purposes; andrographis is an herb from Southeast Asia; and beta-glucan is a type of fiber derived from yeast or mushrooms, also studied for immune support.
The capsules also contain inactive ingredients — cellulose, water, and hypoallergenic plant fiber — which are standard binders and fillers.
Does it work?
Not established
The evidence for these ingredients varies. Andrographis shows the strongest support, rated Possibly Effective for sore throat and ulcerative colitis, and Possibly Effective for osteoarthritis.
Beta-glucan is rated Likely Effective for high cholesterol (dyslipidemia) and heart disease risk. Reishi and astragalus, however, lack established evidence for the conditions they're traditionally used for — the data on file rates them as Insufficient Reliable Evidence for most uses, including immune support, though reishi is rated Possibly Ineffective for high cholesterol.
If you're taking this specifically for immune support, the evidence is not yet clear enough to say whether it works for that goal.
How safe is it?
Well-documented data
These ingredients are generally well tolerated in the short term, though long-term safety data is limited for reishi and astragalus, and product quality varies for andrographis. Common side effects are mild — dizziness, dry mouth, nausea, stomach upset, rash, and headache have been reported — but andrographis can cause more frequent and serious effects at high doses or with extended use, including severe allergic reactions and skin conditions like Stevens-Johnson syndrome.
Reishi may rarely cause insomnia, and astragalus has been associated with dizziness in some studies. The safety data advises against reishi and astragalus during pregnancy and breastfeeding because there isn't enough information.
Andrographis is rated Possibly Unsafe in pregnancy due to theoretical concerns about reproductive effects and should be avoided during breastfeeding as well. Beta-glucan from food is likely fine during breastfeeding, but concentrated supplement doses haven't been well studied.
Meds to double-check
Moderate interaction found
Before taking this product, double-check your medications with your doctor or pharmacist if you take blood pressure drugs, blood thinners (anticoagulants or antiplatelets), immunosuppressants (including after a transplant), lithium, or diabetes medications. All of these Moderate-severity interactions are documented in this product's ingredients.
Andrographis also has Minor interactions with specific NSAIDs (celecoxib, etoricoxib) and the diabetes drug glipizide.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
This product brings together four immune-support ingredients with varying evidence and overlapping medication interactions — most notably with blood pressure drugs, blood thinners, immunosuppressants, and diabetes medications. If you take any prescription medication, especially for blood pressure, bleeding, immune suppression, or blood sugar control, talk with your doctor or pharmacist before starting it.
Pregnant and breastfeeding people should avoid it given the limited safety data, particularly for reishi and andrographis.

Innate Immune Support by Pure Encapsulations Drug Interactions
HelloPharmacist Interaction Report
Pure Encapsulations Innate Immune Support contains four active ingredients — reishi, astragalus, andrographis (AP-Bio), and beta-glucan — and together they interact with a substantial number of medications.
The most serious interaction we have on file is Moderate severity: reishi and andrographis can both lower blood pressure, and when combined with blood pressure medications (antihypertensive drugs), they may increase your risk of excessively low blood pressure (hypotension).
Read the full breakdown — every affected drug type, severity by severity
Altogether, these interactions span 498 individual medications. The remaining concerns fall into several categories.
Reishi, andrographis, and beta-glucan may all interfere with blood thinners (anticoagulant and antiplatelet drugs) and increase bleeding risk. Astragalus and andrographis can theoretically interact with drugs used to suppress the immune system (immunosuppressants), potentially reducing their effect — a particular concern if you take them after an organ transplant or for an autoimmune condition.
Astragalus may also raise lithium levels if you take that mood stabilizer. Reishi and astragalus could theoretically add to the effect of diabetes medications and increase your risk of low blood sugar (hypoglycemia).
Andrographis carries additional Minor-severity interactions with specific diabetes and arthritis drugs (glipizide, celecoxib, etoricoxib).
Beta-glucan from food sources is generally well tolerated, and we hold no documented interactions for it beyond antihypertensive and immunosuppressant drugs. Before you start this product, use the medication checker on this page to look up your exact prescriptions and over-the-counter drugs.

AP-Bio
Anticoagulant/Antiplatelet Drugs
Theoretically, andrographis might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Animal and laboratory studies suggest that andrographis has antiplatelet effects.
Antihypertensive Drugs
Theoretically, andrographis might increase the risk of hypotension when used with antihypertensive drugs.
Animal research suggests that andrographis has hypotensive effects.
Immunosuppressants
Theoretically, andrographis might interfere with the effects of immunosuppressive drugs.
Laboratory research suggests that andrographolide has immunostimulant activity.
Celecoxib (Celebrex)
Theoretically, andrographis extract might increase the maximum concentration and time to peak concentration of celecoxib. The clinical significance of these changes is unclear.
Animal research suggests that andrographis extract taken orally increases the maximum concentration and time to peak concentration of celecoxib but does not appear to impact the area under the curve.
Etoricoxib (Arcoxia)
Theoretically, andrographis might decrease the absorption of etoricoxib, although the clinical significance is unclear.
Animal research shows that andrographis extract, or the constituent andrographolide, taken orally with etoricoxib decreases the bioavailability of etoricoxib. However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
Glipizide (Glucotrol)
Theoretically, andrographis extract might increase the maximum concentration and area under the curve of glipizide; however, opposite effects are seen with the constituent, andrographolide. The clinical significance of this interaction is unclear.
Animal research suggests that andrographis extract taken orally with glipizide in diabetes-induced rats increases the maximum concentration and area under the curve of glipizide. However, the opposite effect is seen with the constituent, andrographolide, in which the maximum concentration and area under the curve are decreased when taken with glipizide.
Reishi Fruiting Body Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
Beta-Glucan
Antihypertensive Drugs
Theoretically, taking beta-glucans with antihypertensive drugs might increase the risk of hypotension.
Clinical research shows that taking beta-glucans may reduce systolic and diastolic blood pressure in some hypertensive individuals.
Immunosuppressants
Theoretically, beta-glucans might interfere with immunosuppressive therapy.
Some clinical research shows that beta-glucans have immunostimulant effects.
Astragalus Root Extract
Antidiabetes Drugs
Theoretically, taking astragalus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research in humans shows that astragalus might have hypoglycemic effects. Theoretically, taking astragalus, especially in combination with other hypoglycemic agents, might increase the risk of hypoglycemia.
Cyclophosphamide
Theoretically, astragalus might interfere with cyclophosphamide therapy.
Evidence regarding the effect of astragalus on immunosuppression caused by cyclophosphamide is conflicting. Some animal research suggests that astragalus reverses cyclophosphamide-induced immunosuppression. However, other animal research shows no effect.
Immunosuppressants
Theoretically, astragalus might interfere with immunosuppressive therapy.
Astragalus seems to stimulate immune function. Theoretically, taking astragalus might decrease the effects of immunosuppressive therapy.
Lithium
Theoretically, astragalus might increase levels and adverse effects of lithium.
Animal research suggests that astragalus has diuretic properties. Theoretically, due to this diuretic effect, astragalus might reduce excretion and increase levels of lithium.
