D-ribose is generally well tolerated short-term — usually up to one month — but long-term safety isn't well studied. The most common side effects are diarrhea, gastrointestinal discomfort, headache, and nausea; some people report lightheadedness, increased appetite, or mild anxiety, though lowering the dose can resolve these.
