This powder contains two active carnitine compounds: L-carnitine and acetyl-L-carnitine. L-carnitine is an amino acid your body uses to move fats into your cells' energy-making factories.
Acetyl-L-carnitine is a modified form that can cross into the brain more easily. The formula also includes natural lemon flavor and stevia extract as inactive ingredients for taste.
Does it work?
Strong evidence
L-carnitine is effective for L-carnitine deficiency. It's rated possibly effective for high cholesterol (hyperlipidemia), heart failure (congestive heart failure), and angina—a chest pain condition caused by narrow heart arteries.
Acetyl-L-carnitine is rated possibly effective for age-related mental decline, Alzheimer disease, alcohol use disorder, nerve damage from diabetes, and depression. Keep in mind that 'possibly effective' means there is some clinical evidence, but it's not as strong as we'd like it to be.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at normal doses, though high doses can trigger stomach upset, diarrhea, nausea, and a fishy body odor. Acetyl-L-carnitine is also generally well tolerated short-term, but long-term safety data is limited.
Common side effects of acetyl-L-carnitine include dry mouth, headache, insomnia, restlessness, and the same fishy odor. Both can cause abdominal pain and reduced appetite.
Seizures have been reported rarely. The safety data for L-carnitine during pregnancy is not established; for acetyl-L-carnitine, there isn't enough information on either pregnancy or breastfeeding safety—talk with your doctor or pharmacist about whether this product is right for you if you're pregnant or nursing.

Acetyl-L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
