What’s inside
Full disclosure
Nattokinase NSK-SD by Pure Prescriptions contains one active ingredient: NSK-SD nattokinase, a protein-digesting enzyme derived from fermented soybeans. The capsule itself is made from vegetable cellulose, and rice flour is included as an inactive ingredient.
Does it work?
Not established
The evidence we have doesn't establish whether nattokinase works for any of its proposed uses—angina, atherosclerosis, cancer, cardiovascular disease, or chronic fatigue syndrome all carry insufficient reliable evidence ratings in our data. That doesn't mean it doesn't work; it means the evidence isn't solid enough for us to rate it one way or the other.
Talk with your doctor about whether this product makes sense for your specific health goal.
How safe is it?
Well-documented data
Nattokinase is generally well tolerated in the short term, but because of its blood-thinning effects, it carries caution for anyone at higher risk of bleeding. Rare cases of hemorrhage—including brain bleeding—have been reported, particularly when combined with other blood thinners.
If you have a history of bleeding problems or take blood thinners, this is something to discuss with your doctor before starting. Not enough safety data exists for pregnancy or breastfeeding.
The safety notes advise against use during both. Talk with your doctor or pharmacist about your individual situation if you're pregnant, planning pregnancy, or breastfeeding.
Meds to double-check
Moderate interaction found
Check before starting if you take blood thinners (anticoagulants) like warfarin or aspirin, or any antiplatelet drugs—there's a Moderate risk of bleeding. Also flag any blood pressure medications you're on, since nattokinase may lower pressure further, creating a Moderate risk of blood pressure dropping too low.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
Nattokinase is an enzyme with blood-thinning and blood pressure-lowering properties. If you take blood thinners or blood pressure medications, you need to check with your doctor or pharmacist before starting this product—the interaction risk is real.
The effectiveness for heart health and other conditions isn't established in the data we hold. Discuss with your own healthcare provider whether nattokinase is appropriate for you and your medications.

Nattokinase NSK-SD by Pure Prescriptions Drug Interactions
HelloPharmacist Interaction Report
Nattokinase NSK-SD by Pure Prescriptions carries documented interactions through its single active ingredient, nattokinase, with blood pressure medications and blood thinners.
The most serious concern is with blood thinners (anticoagulant and antiplatelet drugs)—nattokinase has blood-thinning properties of its own, and combining it with these medications raises the risk of bleeding. There's a documented case of brain bleeding in a patient who took nattokinase alongside low-dose aspirin.
Read the full breakdown — every affected drug type, severity by severity
Nattokinase may also lower blood pressure on its own. When paired with blood pressure medications (antihypertensives), there's a theoretical risk of blood pressure dropping too low.
Altogether, these interactions span 289 individual medications.
Before you start nattokinase, check with your doctor or pharmacist about any medications you're taking—especially aspirin, warfarin, or other blood thinners, and any drugs you take to manage high blood pressure. Your provider can help you weigh the risks and decide if this product is right for you.
