Neem leaf preparations are generally well tolerated for short-term use in adults, though high-quality safety research is limited. The most common side effect when applied to skin is contact dermatitis in people with sensitive skin.

Rare serious effects reported orally include cardiac issues and kidney damage in adults; neem oil in children can cause serious harm, so this form should never be given to kids. One case report described hemolytic anemia (breakdown of red blood cells) in an adult with a specific enzyme deficiency (G6PD) who drank large amounts of neem tea daily.
