Here's the simple version: both neem and acarbose can push your blood sugar in the same direction, which is down. Acarbose works by slowing how fast your meal turns into sugar, so your post-meal numbers don't spike as high. Some small studies suggest neem can also nudge blood sugar lower. Stack the two together and you may dip lower than you intended.

Honestly, the evidence here is thin, mostly lab and animal work plus a few small human studies, so don't panic. Still, if you add neem, keep an eye out for low-sugar signs like shakiness, sweating, dizziness, or feeling foggy. One heads-up with acarbose specifically: if a low does happen, treat it with glucose tablets or juice, since acarbose can blunt how fast table sugar raises your blood sugar back up. Please loop in your pharmacist or doctor before combining them.
