Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version for your headache medicine (Tecnal, Esgic, and similar combos). One of its three parts, caffeine, is cleared from your body mainly by a liver enzyme called CYP1A2. In lab tests, neem leaf extract seems to slow that enzyme down. If that happened in real life, the caffeine could linger longer and pile up a bit, leaving you feeling jittery, wired, restless, or having trouble sleeping, plus a faster heartbeat.
The honest part: this is only seen in test-tube studies, and it has never actually been reported in people. So I wouldn't lose sleep over it (pun intended). Still, if you take concentrated neem supplements regularly, mention it to your pharmacist or doctor before combining the two.

Here's the gist for your headache combo (Tecnal, Esgic, and similar brands). In lab dish studies, neem leaf extract seems to slow down a liver enzyme called CYP3A4, which helps clear the butalbital piece of this medication. If neem blocked that enzyme in your body, butalbital could build up a little, leaving you more drowsy, dizzy, or groggy than usual. None of these are activated by the enzyme, so this isn't a prodrug situation, it's just a possible slowing of breakdown.
The important part: this has only been seen in test tubes, never reported in real people, so the concern is mostly theoretical. Still, since butalbital is sedating, it's worth running your neem product by your pharmacist before combining them.

This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).
All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
