Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.

This combination pill has four ingredients, and the one that connects to neem here is the caffeine. Caffeine is cleared mostly by an enzyme called CYP1A2. In lab dishes, neem leaf extract seems to slow that enzyme down, which could let caffeine linger longer in your system. In real life that might mean feeling jittery, a racing heart, trouble sleeping, or an upset stomach. Honestly, though, this has only been seen in test tubes, never in actual people, so I wouldn't lose sleep over it.

The acetaminophen, butalbital, and codeine pieces aren't really driven by this particular enzyme, so they're less of a concern. Still, since neem can be hard on the body in concentrated doses, run it by your pharmacist or doctor before combining them.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

This combination pill has a few moving parts, but the piece that matters here is butalbital, a barbiturate that your liver partly clears using the CYP3A4 enzyme. In lab dish studies (not in people), neem leaf extract seems to slow down CYP3A4. If that happened in your body, butalbital could build up a bit, which might leave you feeling extra drowsy, foggy, or dizzy, especially when paired with the codeine in this medicine.

I want to be honest though: this is only a theoretical concern from test-tube work, and it has never been reported in real patients. Still, since both butalbital and codeine already cause sedation, it's worth a quick chat with your pharmacist or doctor before adding neem.
